East Harlem Council for Human Services, Inc.
Responsibilities: Provide mental health counseling services and psychosocial support and case management services to patients of the East Harlem Council for Human Services, Inc. (Council)/Boriken Neighborhood Health Center (Boriken). Contribute to performance improvement, staff development, and other programmatic and clinical functions. Some duties include, but are not limited to the following; provide consultation regarding mental health and psychosocial issues to medical and other staff of the Boriken Neighborhood Health Center and other Council programs. Assure that the mental health/psychosocial component of the Boriken Neighborhood Health Center is in compliance with guidelines and regulations of licensing and funding agencies and in keeping with current practices regarding the delivery of mental health and psychosocial support services. Maintain all licensure requirements, as well as perform other functions necessary for the proper care of patients and functioning of the Council.
Qualifications: NYS Licensed Clinical Social Worker, Masters degree in Social Work, Minimum of 5 years experience preferred. Bilingual Spanish strongly preferred. Experience working with patients and families, including individuals with HIV/AIDS, a history of substance abuse, mental illness, domestic violence and families at risk. Dedicated to providing high quality patient-centered services.
