Case Manager- Lee

The Center for Urban Community Services (CUCS), a national leader in the development of effective housing and service initiatives for homeless people, invites applications for a CASE MANAGER position at the Lee, a 207-unit permanent supportive housing program for low income tenants, many of whom have histories of mental illness, homelessness, substance abuse, and /or HIV/AIDS. The Lee is located on the Lower East Side in Manhattan

The Case Manager is responsible for:
1. Working with an assigned caseload of roughly 25-30 special needs and low-income clients
2. Collaborating with colleagues and team mates to meet contractual obligations and program success
3. Utilizing core clinical principles to assist clients in their move towards fuller and more satisfying lives
4. Coordinating with outside providers to ensure holistic care for clients
5. Providing strengths-based advocacy and collaboration with clients
6. Fulfilling CUCS’ core purpose, core values, and vision statement

Experience and Education Required:
• Bachelor’s Degree
or
HS Diploma and 4 years experience in the relevant field
• Appropriate verbal and written communication skills
• Computer literacy
• Familiarity with mental health, homelessness, and/or substance use
• Bilingual Spanish/English preferred
